When you are a new patient your first appointment is always with a Care Coordinator. Why? Our Care Coordinators are experts in helping determine what services would benefit you most, and they will help match you with a provider. Care Coordinators are compassionate, friendly individuals who we staff simply to listen to your needs and help guide you to the right place.
When you leave your Care Coordination appointment, you will have your next steps in your care plan, whether that be to see a psychiatrist, a therapist, or both. Once you see your provider(s) we will create a customized and comprehensive care plan. Our staff and licensed professionals meet you where you’re at – you can be seen virtually or in-person at one of our many offices across Florida.
Preparing for Your First Appointment
Prior to your first appointment with our Care Coordination team (sometimes you will see this referred to as intake) you will receive a note to your email address to create your patient portal login and complete patient paperwork.
If you are unable to complete your intake paperwork prior to your first appointment, your Care Coordinator will walk you through this – even if you have a virtual appointment set up. Just contact us and plan to visit your nearest office to pick up physical paperwork, fill it out, and provide it to our Patient Services Representative at our Front Desk.
For in-person appointments, please arrive 15-20 minutes prior to your scheduled appointment if you have not completed your online paperwork. This gives us time to input your insurance card, new patient forms, patient information and more. Additionally, please make sure to bring a list of any medications that you are taking currently. The first appointment may take a little extra time, but we promise to make this first appointment seamless as possible. Thank you for your patience!
Exploring The Benefits of the Patient Portal
You will now have access to Elite DNA’s Patient Portal, which allows you to have your health information right at your fingertips. You can send secure messages to your Provider, request prescription refills, find your lab and test results, and much more. Plus, the Patient Portal is available 24 hours a day, 7 days a week and 365 days a year! We encourage our patients to sign up for the Patient Portal and utilize it throughout the year to quickly and efficiently manage their healthcare.
